Pre-Assessment Questionnaire

Initially families and adults are asked to complete a questionnaire where it is determined if the program can be of help. Research (published in The British Journal of Occupational Therapy, October 1998) has shown that a score of 7 or more 'yes' answers on the questionnaire indicates that further investigation is advised for children over 7 years of age. If you score over 7 ‘yes’ answers, it is recommended to follow up by booking a free initial consultation.

The below forms are not subject to the 7 ‘yes’ answers, all queries with babies and listening can be addressed even if you have only one concern.
 

Once the form is filled out it will be sent to me and I will email or phone you to make an appointment.
